Enrollment Forms

If interested in enrolling your child, be sure to contact our facility for a tour and policy meeting.

After a decision is made, you may download, complete, and submit the forms in the .pdf document(s) listed below.

The forms can be submitted to our office or via email skyerscdc@bellsouth.net In the subject line please list your Child's Name and  Enrollment Forms.   

 

Things to Remember:

• Check List lists the needed forms

• The file titled Enrollment Package has all forms in one .pdf.

• The file titled Nutrition Package has all nutrition forms in one .pdf.

• A Registration Fee of $100 will be required upon submission of the Enrollment Package. May pay at Office.
